By submitting this survey, I agree that my responses accurately reflect my thoughts, and that I will submit only one survey entry per person. I understand that by submitting both my full name and email address correctly, I am entering a sweepstakes conducted by the Celebrating Asian American Heritage Foundation ("CAAHF", "Foundation") that does not require payment to enter into consideration. I agree that I must be 13 or older to enter the sweepstakes, because it is illegal for us to collect personal information from those under 13 (15 U.S.C. § 91). I understand the Foundation has the right to prune duplicate email entries and names when our random drawing is conducted, and the Foundation shall be the sole arbiter and determiner of the winners. I agree that if I win, I must respond to the email to coordinate our gifting of the card to you, and if I do not respond within 3 days, I will forfeit my prize and the prize will be given to someone else. I understand that, by the nature of sweepstakes, I might not win.

By submitting this survey, I also agree to perpetually release the text I have written in this survey, as well as the ideas and intellectual property, to the Foundation. I agree Foundation is not required to remunerate me for my ideas or intellectual property, and the gift card shall not count as such if I am selected as a winner.

I understand hate crimes or threats thereof are illegal under 18 U.S.C. § 249, and that any illegal or threatening messages will be referred to either the Plano Police Department or the FBI for investigation and potential prosecution.
